This is considered the gold standard in surgical brow lift procedures today. Small incisions are made within the hair. These small incisions allow a tiny endoscopic camera to be inserted. The eyebrow is then gently repositioned and shaped from underneath the skin.

The brow elevator and depressor muscles are the ones that control brow position, and these can be treated at the same time as the brow repositioning procedure The skin is sutured closed at the hairline junction to hide the incisions. Typically, the incision is made so that portions of hair grow through the scar after healing, making it very difficult to see once healed. The common incision types for this style procedure include trichophytic (within the hairline) and pretrichial (just in front of the hairline).
